What is an umbilical hernia?

An umbilical hernia occurs when part of the intestines, fatty tissue, or other abdominal contents push through a weak spot in the abdominal wall, specifically around the belly button. This area is naturally more vulnerable due to the way the muscles form during development. Increased abdominal pressure from activities like heavy lifting, straining, pregnancy, or gaining weight can push tissue through this weakened spot, creating a noticeable bulge near the navel.

Umbilical hernias are common in both men and women, and while they can occur at any age, certain factors such as pregnancy, obesity, or previous abdominal surgeries may increase the risk.

Symptoms

  • A noticeable bulge near or around the belly button that may become more visible when standing or straining
  • The bulge may flatten or disappear when lying down
  • Discomfort or pain around the navel area, especially when coughing, lifting, or bending
  • Sensitivity or tenderness around the belly button
  • A feeling of pressure, fullness, or aching in the abdomen, particularly when engaging in physical activities.

Side note: If you notice a bulge between your belly button and ribcage, it may be an epigastric hernia, caused by tissue pushing through a weak spot in the upper abdominal wall. Similar to umbilical hernias, these can also be repaired without the use of mesh.

The right approach depends on several factors, including the size of your hernia, your symptoms, and your lifestyle.

Since hernias cannot heal naturally, surgery is the only definitive way to repair the weakened area. There are two main surgical approaches:

  • Mesh-Based Repair (Common in your local hospitals, but carries risks of chronic pain and foreign body reactions)
  • Mesh-Free Repair () The natural alternative, reinforcing your own tissue without implants.

When is surgery recommended?

For small hernias that cause no pain or discomfort, some people choose to monitor their condition rather than undergo immediate surgery. This is known as “watchful waiting.” However, hernias do not heal on their own—over time, they tend to grow larger and can become painful or even dangerous.

Some people try hernia belts or trusses to hold the bulge in place. While they may provide temporary relief, they do not fix the hernia and can even cause discomfort or skin irritation with prolonged use.

The operation explained

Our mesh-free hernia repair technique directly addresses the issue by carefully stitching and reinforcing the weakened groin area, eliminating the need for plastic mesh. This method restores your natural tissue structure and is performed as out-patient surgery, meaning you’ll have the procedure and return to rest in the comfort of your own accommodation on the same day.

Start of the operation

After administering the anesthetics, we make a small horizontal incision of approximately 5 cm directly above your hernia, to access the hernia bulge.

The hernia sac is gently reduced back into the abdomen and the weakness in your abdominal wall is closed with sutures, using the Mayo technique.

Finally, the operation wound is closed using sutures and surgical glue. In all, the operation takes about 30 to 45 minutes to complete. After full recovery, a minor scar of 3 to 5cm is left.

Criteria for umbilical hernia surgery without mesh

At BioHernia, we specialise in mesh-free repairs for umbilical hernias, but not all hernias are suitable for this approach. Every patient is unique, and we take a personalised approach to determine whether a non-mesh repair is the right solution for you. Factors like hernia size, body composition, and abdominal wall conditions are carefully evaluated to ensure the chosen surgical method is safe, effective, and tailored to your needs.

Why the criteria are stricter

The criteria for umbilical hernia surgery are stricter because the tissue near the belly button is naturally weaker than in other areas, such as the groin. This means the repair must be performed with greater precision to avoid complications or recurrence. Additionally, factors like abdominal pressure and the risk of tension on the repaired tissue are higher, requiring careful patient selection to ensure the success of a non-mesh repair.

One of the most important criteria for a mesh-free repair is the size of the hernia. Larger hernias increase the risk of recurrence, which is why we only repair smaller umbilical hernias without mesh. Here’s what we look for:

  • The neck of the hernia (the opening in the abdominal wall) must not exceed 10 mm.

  • Hernias with a neck size between 10 mm and 20 mm may still be eligible, but this depends on an individual assessment.

 

A simple rule of thumb: If the hernia bulge is larger than a peanut when pushed out, it may not be suitable for mesh-free repair.

Excess weight, particularly a large abdominal circumference, places additional strain on the abdominal wall and can compromise the success of a non-mesh repair. Patients should maintain a healthy body weight to be eligible. If necessary, we may recommend addressing weight-related concerns before surgery to improve outcomes.

The presence of a significant diastasis recti (a separation of the abdominal muscles) can affect the integrity of the abdominal wall, making a mesh-free repair more challenging. If a diastasis is identified, the decision to proceed will depend on a detailed assessment by our surgeons. In some cases, additional procedures may be required to address the diastasis alongside the hernia repair, or the use of a Phasix resorbable mesh may be necessary to ensure long-term strength and stability.

1 – Pre-operation

The day before your operation, you’ll visit the clinic for final pre-surgery checks. During this visit, you’ll meet your surgeon and anesthesiologist, who will walk you through the procedure and answer any remaining questions you may have.

Will the surgeon examine my hernia?

The following weeks

As you move into the second week and beyond, your recovery will gradually progress. Swelling and bruising should subside, and your energy levels will start to return. While it’s important to continue avoiding heavy lifting or intense activity, you can begin to reintroduce more of your usual routine during this period.

Weeks 2 and 3

You can slowly increase your activity level, incorporating light household tasks, short walks, or gentle cycling on a stationary bike at home or in the gym. These are all great ways to improve blood flow and rebuild strength without placing strain on the surgical site. Be sure to listen to your body and avoid pushing yourself too hard during this stage.

Weeks 4 to 6

By this stage, your recovery is well underway, and you’ll feel significantly stronger. You can confidently return to your regular activities, including more dynamic exercises like running or competitive sports. For those who enjoy weightlifting or more strenuous workouts, this is the time to start reintroducing these activities. While the repair is strong enough to handle heavier lifting, it’s still important to ease into it, gradually increasing intensity to allow your body to fully adapt. Most patients find they’re back to feeling like themselves by the end of week 6, ready to embrace an active lifestyle without limitations.

Resuming normal life

Most patients can return to office work or light-duty tasks within 1–2 weeks. However, for physically demanding jobs, it’s best to wait until 4–6 weeks after the procedure.

You can resume light activities, such as walking, within the first week after surgery. For more strenuous exercises like weightlifting, running, or intense workouts, it’s best to wait 4–6 weeks and ease back into them gradually, paying close attention to how your body feels.

Can an umbilical hernia heal on its own?

No, an umbilical hernia cannot heal on its own. Without treatment, a hernia can worsen over time, leading to complications such as pain, increased swelling, or even strangulation, where the blood supply to the affected tissue is cut off, and you have to undergo emergency surgery.

Umbilical hernias occur due to a weakness or defect in the abdominal wall muscle around the navel (umbilicus).

  • In infants: The muscles and connective tissues at the belly button area are not fully fused at birth. As the infant grows, these tissues typically come together and close the small opening, but if they don’t, it can lead to a protrusion of abdominal contents, forming a hernia.
  • In adults: Factors that increase intra-abdominal pressure can contribute to the formation of an umbilical hernia. These factors include:
    • Obesity
    • Pregnancy (especially multiple pregnancies)
    • Chronic coughing
    • Chronic constipation or straining during bowel movements
    • Heavy lifting or activities that frequently strain the abdominal muscles
